PIECES Team Mobilization Job Aid - Free Download

PIECES Team Mobilization Job Aid – helps the Team move forward with Investigations, Interactions, Interventions that have been identified in Q.3 (What are the actions?) of the PIECES 3-Question Template.  It has been designed to focus attention on how  the actions will be shared, who needs to be involved, what is the plan for monitoring so the Team knows what care strategies need to be maintained or  modified, and how the Team partner during transitions in care. Remember, implementation of the care plan cannot be left to chance!

If pharmacological interventions are part of the action plan, using Detect-Select-Effect will help the Team communicate information that has been gathered using the 3-Q Template (including evidence from assessment tools) to meaningfully inform decision-making.
