Team PIECES™ in Action Resource Package!
We know that best possible care is realized when the Team acts together! The PIECES™ approach in action is driven by meaningful conversations that focus the Team in shared solution finding using the PIECES™ 3-Question Template. The Team always begins with the Person and Care Partner.
The Team PIECES in Action! Resource Package has been developed to support all PIECES Practitioners, and to further enhance Team collaboration when using the PIECES™ 3-Question Template. Together, these resources will raise awareness about the PIECES™ approach and the critical role allTeam members have in shared support of the Person. Wherever the PIECES™ approach is implemented, they can be a catalyst for the Person and Care Partner(or any Team members) to participate in the shared solution finding. These conversations can occur in-the moment, Team huddle, care conference, in-person, on the phone or virtually.
PIECES™ Information Sheet
- Creates awareness of the PIECES™ approach and how it can help the Team to understand changes in a Person’s behavioural expressions, especially when the Person is living with complex health conditions.
- Provides conversational questions, which have been clearly formatted using the PIECES™ 3-Question Template, to more fully engage Team members in the collaborative assessment and supportive care planning.
Important: The PIECES™ 3-Question Template has not changed and this new Resource Package does not replace the core PIECES™ tools (Job Aids, Resource Guide, etc.)! It remains essential that the PIECES Practitioner, in collaboration with the Team, continues to use the PIECES™ Job Aids and other resource materials to guide the clinical assessment.

The PIECES™ acronym, which will encourage Team conversation that recognizes all aspects of the Person’s health and well-being, stimulate thinking, and sharing of information unique to the Person, always considering:
- Physical, Intellectual, and Emotional health, Capabilities both strengths and areas of need, physical and social Environment, and the Person’s Social self, including their life story, cultural, spiritual, sexuality and gender identity.
- The essential need to work together as a Team, reinforcing that everyone sharing what they know is critical to inform assessment, care planning, and moving forward together.
PIECES™ Pocket Card
An easy to carry resource that:
- Provides the conversational questions, clearly formatted using the PIECES™ 3-Question Template, to engage the Team and focus the shared solution finding using a common language and approach.
Encourages all Team members to contribute more meaningfully to the clinical assessment and care planningusing a common language and approach.
The resources were developed as a package. They complement one another, support Team relationship building in settings where the PIECES™ approach is implemented, and reinforce that the PIECES Practitioner is not alone. They can be used to help further mobilize communication and planning at the Team and organizational levels, and including external partners.
Examples of how to use this Resource Package in practice:
- Introduce the Resource Package proactively so it can be quickly mobilized when needed e.g., include in an organization’s information/welcome packages wherever the Person is being supported.
- Refer Care Partners (and any Team member) to the Resource Package when concerning behavioural expressions emerge. This will validate the importance of their contribution and act as a catalyst for shared solution finding conversations.
- Integrate into orientation for all new staff members.
Working together to value and ensure the voices of all Team members are heard is the PIECES approach in action!
