The PIECES journey began in 1997 and has evolved to address changing needs of Teams, organizations and systems. Today, Pieces Canada remains committed to the development of educational programs which are grounded in proven principles and practices of adult learning and performance improvement, building upon existing knowledge of health care practitioners and providing opportunity to apply new skills while learning the PIECES approach.
Our educational programs, and recognized best practice PIECES approach, play a critical role in enhancing capacity of health practitioners across the continuum of care to guide collaborative engagement, shared assessment and supportive care and the fostering of an integrated approach to system wide collaboration.
We pride ourselves on the quality of our collaborative relationships with PIECES Learners, PIECES Practitioners, and PIECES Educators, and partnerships with organizations. The evolution of the PIECES approach and education programs has and always will be informed by lived experience, practice based experience and research.
Pieces Canada partners with organizations, health regions and governments to inform the development of policy supporting the implementation, sustainability and spread of the PIECES approach as one component of enhancing health care for older adults living with complexity.
