From Spring 2023 to Fall 2025, Pieces Canada had the pleasure of partnering once again with Western University, Health Sciences, Registered Practical Nurses of Ontario (WeRPN), Vision 74, and Copper Terrace Long-Term Care homes located in Ontario. The goal of the research was the co-design of a Toolkit for LTC homes implementing virtual team care planning using the PIECES Approach. This work is a continuation of the research project COVID-19: Implementation of virtual PIECES™ for LTC resident care planning with family to build and sustain team collaboration and workforce resilience,” which resulted in several publications.
The Toolkit provides practical information to help Care Partners/Families prepare for a Care Conference in long-term care whether meeting in-person or virtually. It can also assist long-term care homes and their PIECES Practitioners to implement virtual team care planning using the PIECES 3-Question Template to support person-centred care.
